Espoused as early as the 1960's by the American Board of Pediatrics as well as other medical organizations, this model of care relies on continuity, accessibility, as well as "one-stop"coordinating for medical care.

It relies on a familiarity with the patient as the foundation for enhancing communication to insure that the physician practices to his full potential in aiding the patient improve their quality of life. When practiced in a non intimidating setting, the model empowers the patient to contribute to their own well being.

 

Recently, with the Healthcare Reform Act of 2010, the recognition of the importance of the Medical Home model has gained prominence. Berlin Pediatric Associates has always coveted this importance and stands poised to continue to implement any recommendations which improve patients' access to care on all fronts.
We are committed to improving the empowerment and partnership of parents in the care of their children. To that end, we accept all forms of Connecticut Medicaid for children, as well as provide a sliding scale for families of limited means

Berlin Pediatric Associates was chosen as one of the initial twenty five practices in the State of Connecticut to partake in the NCQA certification as a PCMH (Patient Centered Medical Home) practice. Certification was completed on August 1, 2011 and carries with it a distinction that few medical practices have achieved. We are proud to be one of the few exclusively pediatric practices to achieve this recognition nationwide.
