AFTER HOURS: Parents
can call the emergency number (860-418-7810) FOR EMERGENCIES ONLY and
speak directly with Dr. LoPreiato. We prefer that routine calls be handled during regular office hours.
Our office shares call coverage with Dr. Barbara Ziogas of 1 Forest Park, Farmington, Connecticut. When she
is on call for our office, parents should also dial 860-829-7337 and they will be directed appropiately.

Questions: You may call between the hours of 7:00AM and 4:00PM during
regular hours to speak to either Dr. LoPreiato or the office staff directly. Dr. LoPreiato will make every effort to
address your routine questions as soon as time permits. Please ask to speak to Dr. LoPreiato directly if you feel an issue
is urgent.

Insurance:
We recommend you notify your health insurance carrier of the selection of your pediatrician in advance so as to insure maximum
healthcare coverage. Most carriers require approximately 30 days advance notice of the birth of your child. Please be familiar
with the stipulations of your policy. We accept most insurance companies. Please contact either your carrier or our office
to see if we accept your plan. Please be familiar with your policy's benefits and limitations, like deductables and copayments.

A AND B PARENTS PLEASE NOTE: YOUR INSURANCE DICTATES THAT YOU MUST DESIGNATE A PEDIATRICIAN BEFORE BEING
SEEN; THEREFORE WE CANNOT SEE YOUR CHILD BEFORE YOU HAVE DESIGNATED US AS YOUR CHILD'S PEDIATRICIAN. MEDICAID PARENTS: PLEASE CALL
US WITH YOUR CHILD'S GRAY CARD INFORMATION BEFORE BEING SEEN. Copayments must be paid as stipulated by your contract with the insurance company (often at
the time of service). You may pay by cash, check, or credit card.

Consultations:
From time to time, we may refer your child to a subspecialist. Please follow-up on your referral in a timely fashion as it
is often difficult for us to remind you to make the appointment. The subspecialist visit is frequently covered differently
by your carrier. If you have concerns about the validity of a particular referral, please do not hesitate to discuss your
reservations.

Fees: Fees for services are based on the prevailing scale
for the area. Though payment is expected at the time of service, we fully recognize problems and hardships that may arise.
Please feel free to discuss them with us.
In cases of divorced or separated parents, or in the event of a third party
bringing the child to the office, please inform the person bringing the child to the office of these obligations.
A
copy of the receipt can be used to obtain reimbursement. A fee schedule is available upon request.
