ABOUT OUR PRACTICE:

 

       Berlin Pediatric Associates was founded by "Dr. Matt" LoPreiato in 2005; Dr. Matt originally practiced in Pittsford, NY for 13 years after completing his internship and residency in Rochester, NY at Strong Memorial Hospital (now Golisano Children's Hospital). He has incorporated the Rochester model of clinical pediatric care here in Connecticut for over a decade. The motto and philosophy of the practice is embodied in "promoting the full potential of your child through continuity." It is a principle we fervently adhere to as it promotes a superior partnership between the physician and parent. Dr. Matt sees each patient at every visit. He takes call every week night as well as two weekends a month for the practice. He shares call with Farmington Pediatrics and Adolescent Medicine the other two weekends of the month.

CARE OUTSIDE OF OUR PRACTICE:

​

We insist that you call us first before visiting any other medical site; in this way we can insure receipt of communication from another provider, including a report of your visit, diagnosis, and medications dispensed. Additionally we can aid the other provider by providing information on past medical history, medications, allergic reactions, as well as a specialist's interventions. We encourage you to have them contact us at the time of your visit.

      

 

We are affiliated with Connecticut Children's Medical Center, Hartford Hospital, and The Hospital of Central Connecticut. Dr. LoPreiato is Board Certified by The American Board of Pediatrics and is a Fellow of the American Academy of Pediatrics.

Dr. LoPreiato was selected as a Healthcare Hero Finalist in 2011 for "Outstanding Achievement in the Healthcare Field" by the Hartford Business Journal.

 

FYI:

EHR REIMPLEMENTATION: We have recently completed implementation of Practice Fusion, our 2nd EHR; This platform allows us to promote our practice portal via "Patient Fusion". Once you are registered, notification of all upcoming appointments are sent to you via your preferred email and/ or text. You may also access our instant messaging feature via the Patient Fusion portal.              

SCHOOL, DAYCARE AND CAMP FORMS: Please remember to bring these in for completion at the time of your visit. SCHOOL FORMS MAY BE DOWNLOADED AT THE "PARENTS" TAB OF OUR WEBSITE.

WAITNG LIST: As a result

of embracing a true medical home model, we find it necessary from time to time to limit the number of new patients we can accept; In such instances, we will provide a space on our waiting list. Periodically, we notify parents of spaces that open up in the practice.

 

 

 

THE MEDICAL HOME:

Espoused as early as the 1960's by the American Academy of Pediatrics, as well as eventually by other organizations, this model relies on true continuity, accessibility as well as "one-stop" coordination of medical care. At Berlin Pediatrics, it relies also on a familiarity with the patient as the foundation for enhancing communication to insure that the physician practices to his full potential in aiding families improve the quality of life for their children. When practiced in a non-intimidating setting, this model empowers families to meaningfully contribute to a child's care. With the passage of the Healthcare Reform Act of 2010, the recognition of the importance of this model has gained prominence. Berlin Pediatric Associates has always coveted this importance, and stands poised to continue to implement any recommendations which improve patients' access to care on all fronts. We are committed towards improving the empowerment and partnership of parents in the care of their children. To that end, we accept all forms of Connecticut Medicaid insurance, and provide a sliding scale to our patients lacking health insurance.

​NCQA RECOGNITION:

Berlin Pediatrics was chosen as one of the initial twenty five practices in the state of Connecticut to partake in NCQA certification as a PCMH (Patient Centered Medical Home) practice. We were told that we were the first solo pediatic group to complete Level 3 certification in 2011. We are proud to achieve this recognition status. Recertification was achieved in 2014, 2017, 2020, as well as yearly since.

 

 

OFFICE POLICIES:

 

Office Hours: 7 am - 4 pm Monday through Friday 

                        8 am - 11 am on selected Saturdays

 

AFTER HOURS:

Call is taken by Dr. LoPreiato on all weekdays and 2 weekends per month. After hours, please call 860-829-7337 to obtain the emergency number to contact Dr. LoPreiato. On the remaining 2 weekends per month, call is taken by Farmington Pediatrics and Adolescent Medicine, 1 Forest Park, Farmington, CT. You may reach them by calling 860-829-7337 to be directed to their answering service.

 

After hours calls are expected to be reserved for conditions that cannot wait until the following morning. All other inquiries (i.e. appointments, chronic stable conditions, non-emergent questions) should be reserved for office hours (7am-4pm). Please ask to speak to Dr. LoPreiato directly if you feel an issue is urgent during office hours.

 

Patients are seen after-hours by Dr. LoPreiato in the office at his discretion after speaking with the parents; Farmington Pediatrics and Adolescent Medicine sees patients after-hours when cross-covering similarly in their office. House calls are implemented per arrangement with Dr. LoPreiato.

 

Copays are expected at the time of service. Please make appropriate accomodations prior to your visit. You may pay by cash, credit card or check.

 

Consultations: From time to time, we may refer you to a subspecialist. Please follow-up on your referral in a timely manner; we will make every effort to implement and remind you of this visit. The subspecialist visit is frequently covered differently by your carrier. If you have any questions regarding the validity or outcome of any particular referral, please do not hesitate to discuss your reservations.

 

Insurance: We recommend that you notify your insurance carrier of the selection of your pediatrician in advance of your delivery so as to maximize healthcare coverage. Most carriers require approximately 30 days advance notice of the birth of your child. Please be familiar with the stipulations of your policy.

 

We accept most commercial insurance, as well as state Medicaid. Please contact your carrier, or our office to verify this. Please be extremely familiar with your policy's benefits and limitations, like deductibles and copays. We don't like surprises, and we suspect you don't either!

 

Huskey A and B patients: PLEASE NOTE THAT YOUR INSURNCE REQUIRES THAT YOU DESIGNATE A PEDIATRICIAN BEFORE BEING SEEN; THEREFORE, WE CANNOT SEE YOUR CHILD UNLESS YOU HAVE DESIGNATED US AS YOUR CHILD'S PEDIATRICIAN. ALSO BE CERTAIN TO BRING YOUR CHILD'S HUSKEY CARD INFORMATION (ID NUMBER) BEFORE BEING SEEN.

 

FEES: fees for service are based on thr prevailing scale for the area. Though payment is expected at the time of service, we fully recognize that problems and hardships may arise; please feel free to discuss them with us. A copy of your receipt can be used to obtain reimbursement. A fee schedule is available upon request.

 

SPECIAL FEES COMMENCING 1/1/2017: DUE TO THE ADDITIONAL STAFF BURDEN PLACED UPON THE COMPLETION OF PAPERWORK FOR SCHOOL, CAMP, DAYCARE OR ABSENCE EXCUSES, A FLAT FEE OF $25.00 PER PATIENT PER YEAR (FOR AN UNLIMITED NUMBER OF FORMS) IS REQUESTED .
